Краткое описание проектаThe purpose of the project is development, manufacturing and clinical tests of the tool for urological surgery, allowing to develop new technology of organ saving operations, to reduce time of operations, to reduce their severity and to relieve consequences, transfer of development to industrial production.
Project manager is Vladimir S. Topunov, Deputy Head of Department, SUE RFNC-VNIITF.
The project will be performed at SUE RFNC-VNIITF facility site and in CROC under the supervision of Karnauh Peter Alekseevich, Candidate of Mathematical Sciences, CROC urology department manager.
CROC has an extensive experience in surgical treatment of oncological and urological diseases. The task of development of two types of tools for urological surgery was formulated based on this experience..
SUE RFNC-VNIITF has an extensive experience in development of various types of engineering systems, including medical ones (Center of neutron therapy, X-ray tomograph). Institute can develop the design and technological documentation for the proposed medical tool, has specialists and process equipment for its manufacturing.
Project implementation assumes performance of two main tasks:
Task 1. Development of the clamp for kidney operations and its clinical tests in CROC.
Task 2. Development of suturing device to suture urethra with bladder and its clinical tests in CROC.
Currently increasing interest of clinicians to the problem of organ saving operations at renal disease is explained by the following circumstances:
1.1. Necessity of operative treatment of patients with a tumour of a single kidney (or single functioning kidney).
1.2. Achieved recently encouraging results of organ saving operations at small tumours.
Clamping of the main renal vessels cannot be long, manipulation time at disconnected blood vessels is limited by 20-30 minutes, that’s why development and introduction into clinical practice of kidney clamp, providing strictly dosed pressure on its tissues and allowing operation of resection on kidney without clamping of the main renal vessels (without ischemic impact on the whole organ), is quite urgent.
Requirements of convenience of sterilization and possibility of the further serial production will be fulfilled at the development of the design of kidney clamp.
Application of the kidney clamp during clinical tests is to be recorded by videocamera for subsequent demonstration to the experts.
Currently number of cisprostatectomy and prostatectomy operations performed in clinics goes up. Making urethras anastomoses with bladder (or the urinary reservoir) has special importance for these types of intervention. Use of machine stitch (suturing device) can considerably simplify formation of such anastomoses and reduce time of operation.
Small diameter of urethra is the reason of absence of the suturing device similar to rectum suturing tool in Russia and in CIS countries.
Suturing should be performed by the compression system connected with Pezzer catheter.
Compression system is extracted through a fistula together with Pezzer catheter in a time required for union of tissues (10-14 days). Compression system will provide constant pressure on union tissues.
The design of suturing device for suture urethra with bladder will ensure reliable sterilization.
The opportunity of serial manufacturing will be stipulated at development of the tool for urological surgery and subsequent organization of batch production and distribution of this tool among hospitals of Russia and CIS countries where similar operations are performed is expected.
