Project summary

The project is developed proceeding from difficult epidemic and epizootic tuberculosis situations in Tajikistan. For today the tuberculosis takes a leading place in structure of people pathology in Tajikistan. Sickness rate has increased by 28 % and has reached 134 on 100 thousand populations. Thus special concern causes high frequency of occurrence of fibrous-cavernous forms of this pathology among for the first time revealed patients which specific gravity in tuberculosis structure has increased in 5 times for last years and death rate has increased on 25 %.. In various regions of Tajikistan the tuberculosis is extended is not identical. Some areas concern to endemic zones and hence there are extremely-high indicators of disease in these areas in comparison with other areas of Tajikistan. It is possible to carry Vose region of Hatlon area to unsuccessful region on tuberculosis. Even in days of existence of Soviet Union in this area there were the highest indicators of disease on the given pathology. According to medical service of republic there are the tubercular patient almost in everyone families in some settlements of this area, there are 2 or 3 members of a family are ill in some families. This area is characterized by unsuccessful epizootic conditions among animals and birds. As a whole epidemic and epizootic situation on tuberculosis in republics is the disturbing. It demands working out of new, modern, highly effective methods of diagnostics, therapy and preventive maintenance of the given pathology from people and animals. Proceeding from above stated the given project is directed on carrying out of the complex analysis of microbiological and genetic properties of tuberculosis germ at people, animals and birds; workings out of effective and high-sensitivity methods of mycobacterium differentiation; tuberculosis diagnostics at an early stage and improvement of quality of actions directed on decreasing of indicators of disease among people and animals, and this is the purpose of the offered project. For achievement of the purpose and the decision of planned problems three leading centers of science of Republic Tajikistan will participate in the Project: the Tajik State Medical University (TSMU), as the Leading research institute; the Tajik Research Veterinary Institute (TSRVI), as Institute-participant 1 and Central Asian Institute of Foot and Mouth Disease (CAIFMD) as Institute-participant 2. At this stage of social and economic development of Tajikistan, ТSMU is a leading centre of science of the country. More than 70 professors and about 400 candidates of medical sciences work in this centre. There are competent experts on chairs of microbiology, virology and immunology and a tuberculosis who have developed the given project work. There are the laboratories equipped with the modern equipment on bases of these chairs. One direction of research work of TRVI and CAIFMD is studying of tuberculosis at animals and birds and working out of instructive-methodological educational aid directed on improvements epizooty of the given pathology in republic. These scientific research institutes also have competent experts of this area of a science and well equipped laboratories.For achievement of this purpose participants of the project developed tasks directed on the analysis of tuberculosis at people and animals, studying etiology structures of tuberculosis and microbiological properties, including antibioresistance of mycobacterium strains. Revealing of poly resistant strains and drawing up of a databank about frequency of occurrence, prevalence and monitoring of these strains. Carrying out of the molecular-genetic analysis of various kinds of Mycobacterium. Studying and findings-out of mechanisms of stability and the possible intrachromosomal gene reorganizations (mutation) leading to occurrence polyresistant strains.In our opinion achievements of the purpose of the given project by means of the decision of tasks will bring the mite in struggle against a tuberculosis in Tajikistan and will enrich a modern science with new data about genetic and the mechanism of occurrence polyresistant strains of Mycobacterium.
